
The 21st Central American and Caribbean Games took place in Mayagüez, Puerto Rico, from 18 July 2010 to 1 August 2010.

The fifty-third edition of the Caribbean Series was played in 2011. It was held from February 2 through February 7 with the champions teams from Dominican Republic, Mexico, Puerto Rico and Venezuela. The format consisted of twelve games, in a double round-robin format with each team facing each other twice. The games were played at Isidoro García Stadium in Mayagüez, Puerto Rico.

The 2011 Central American and Caribbean Championships in Athletics were held in Mayagüez, Puerto Rico. The event served as classifiers for the 2011 World Championships in Athletics and took place from July 15–17, 2011. It was the fourth time Puerto Rico hosted the event; the first time in Ponce in 1975, and later in San Juan in 1989 and 1997.

Caguas Sporting FC is an association football team that plays in Caguas. They currently play in the Liga Puerto Rico.

Club Deportivo Barbosa is an association football team that plays in Rio Grande. They currently play in the Puerto Rico Soccer League.
El Mani Pavilion Pabellon De Boxeo El Maní is a pavilion in El Mani, Mayagüez, Puerto Rico. At the site there was a covered basketball court; this was demolished to make way for the new building. Its construction cost $3,500,000. It was to host events of the 2010 Central American and Caribbean Games. Boxing and mixed martial arts events are held at the Pavilion. A jump roping competition with over 400 participants was held at the Mani Paviliion in 2019.

The Association football competition at the 2010 Central American and Caribbean Games were set to start 21 July, although qualification took take place beforehand. All participants had to be born on or after 1 January 1989. On 13 July 2010 the organizing committee for the games (Comaz) announced that the event would not be held because CONCACAF had threatened to sanction countries that participated.

Indias de Mayagüez is the Professional Female volleyball (LVSF) team of Mayagüez, Puerto Rico. The team's home court is the Palacio de Recreación y Deportes.

Indios de Mayagüez is a Puerto Rico professional basketball team. The team plays in the Baloncesto Superior Nacional (BSN). The Indios play their home games in the Palacio de Recreación y Deportes since 1981. The franchise first entered the league in 1956, and Mayagüez has been its home city for all but five of the team's active seasons given that the franchise has gone through several hiatuses in its history. Along with the Atléticos de San Germán, the Indians have been the only BSN team permanently based in the western part of Puerto Rico, after the Tiburones de Aguadilla relocated to Santurce and became the Cangrejeros de Santurce in 1998.
Estadio Isidoro García is a baseball stadium in Mayagüez, named after Isidoro "El Cholo" García, a local pitcher who threw the first ever no-hitter at a final series game in the Puerto Rico Baseball League. The stadium is located south of the city, at road PR-63, also named the Avenida Colby William Dunscombe. The site lies north of the Mayaguez Athletics Stadium and across the street from the Parque del Litoral.
The Mayagüez Athletics Stadium is 12,175 capacity stadium in Mayagüez, Puerto Rico opened in 2010. The stadium is owned by City of Mayaguez, and operated by Puerto Rico Sol. It hosted the athletics and soccer games of the 2010 Central American and Caribbean Games. In 2020 the stadium will host invitational tournament Puerto Rico Cup. The stadium has a soccer field based on the requisites of International Federation of Association Football, it will also have a 400-meter track which complies with the parameter of the International Association of Athletics Federations.

Mayagüez Football Club is an association football team that plays in Mayagüez. They currently play in the Liga Puerto Rico.

Metropolitan FA is an association football club that plays in the Puerto Rico Soccer League. The team is currently based in San Juan, Puerto Rico, where they currently play their home games at Parque Urb. Reparto Metropolitano.

The Palacio de Recreación y Deportes German Wilkins Vélez Ramírez is a coliseum located in the city of Mayagüez, Puerto Rico. It has capacity for 5,500 spectators. It was remodeled in preparation for the 2010 Central American and Caribbean Games. In March 2008, José Guillermo Rodríguez, mayor of the city, announced in a concert given by Wilkins that after the Central American and Caribbean Games the arena would have the name of Germán Wilkins Vélez Ramírez, commonly known as Wilkins. Wilkins inaugurated the arena with a concert 20 years before. The arena also has an acoustic conch outside, it bears the name of another Mayagüez native Frankie Ruiz.

The Rafael A. Mangual Coliseum is a sports arena in Mayagüez, Puerto Rico. It was inaugurated in 1974 and was designed by Henry Klumb. The Coliseum is managed by the Physical Education Department of the University of Puerto Rico at Mayaguez.
